You are quite correct, they are all in the d/b. I should have looked first before posting. The only one that I would query is 'ABB' for Luxaviation Belgium. That code has been allocated to Air Belgium a new start up or, I should say an old start up that has been resurrected. It is shown as such on the FAA site.
There would appear to be no specific ICAO code for Luxaviation Belgium at this time.

Ceased operations :

FAT - ASL Airlines Switzerland.
I also believe that in an effort to streamline the operation that ASL Airlines Spain is to be wound up over the next few months.

And we also have in the logos folder ICAO code 'JAE' for Jade Cargo International which ceased operations on the 30th December 2011 !
Who says we are not up to date with logos :)

This Company is now part of Integra Air whose logo we have already against 'BXH' so I guess it's up to our Canadian friends who are most likely to see their aircraft which logo they use.

The Bar XH website does mention itself mainly in the past tense, but here's their logo, which will need renaming if folk prefer it to the Integra Air logo
